CPST SERVICE

These are measurable on or off-site face-to-face or telephone or video conferencing services to address the individualized mental health needs of the client in order for the individual to succeed in the community. 

The services are focused on the individual’s ability to identify and access needed services through natural support systems, formal community systems, outreach and advocacy; show improvement in school, work, or family through symptom monitoring, assist in eliminating barriers to employment or education, and crisis management/stabilization; as well as integrate and contribute positively in clients own community or environment.

CPST activities

CPST activities includes ongoing assessment of needs, assistance in achieving personal independence in managing basic needs, and facilitation of further development of daily living skills. 

Case coordination is an integral part of CPST services. CPST services will be provided by licensed social workers or licensed independent social workers and supervised by licensed independent social workers with supervision designation.
